Online Account Application

Complete and SUBMIT the following contact information to begin your account application request with the Ottawa Cooperative Association.

Once the online form has been submitted, please follow the directions below to finish the required documentation to open an account. DOWNLOAD the application form, complete, and return to the Ottawa Coop. Complete instructions below.

The completed application can be returned to the Ottawa Coop in any of the following ways:

  2. Via fax at 785-242-5057
  3. Via mail at P.O. Box 680, Ottawa, KS 66067
  4. Or in person at your local Ottawa Co-op branch

Our office will contact you once your application has been reviewed to inform you that your account has been approved, set up, and is ready to be used.
